Foundayo (orforglipron) is the newest GLP-1 medication on the market, and it breaks one of the biggest rules of the category: it’s a once-daily pill you can take at any time of day, with or without food, and with no water restrictions. Approved by the FDA on April 1, 2026, it gives people who would rather not inject a real alternative to weekly shots. Here’s what it is, how it works, what the trials showed, what it costs, and how to stay consistent once you start.
What is Foundayo (orforglipron)?
Foundayo is Eli Lilly’s brand name for orforglipron, a GLP-1 receptor agonist approved for chronic weight management in adults with obesity, or adults who are overweight with at least one weight-related condition. Like Wegovy, Zepbound, and Ozempic, it works by mimicking GLP-1, the gut hormone that tells your brain you’re full, slows stomach emptying, and turns down food noise.
The difference is what the molecule is made of. Every other GLP-1 you’ve heard of is a peptide, a fragile chain of amino acids that stomach acid destroys, which is why they’re injected or, in the case of the Wegovy pill, taken on an empty stomach with strict timing. Foundayo orforglipron is a small-molecule, non-peptide drug. It survives digestion on its own, so it can be swallowed like any ordinary tablet.

How Foundayo compares to the Wegovy pill
Both are daily oral GLP-1s for weight loss, but the day-to-day experience is different:
- Timing rules: The Wegovy pill (oral semaglutide) must be taken on an empty stomach with no more than 4 ounces of water, followed by a 30-minute wait before food, drink, or other pills. Foundayo has none of those restrictions.
- Weight loss: In separate trials, oral Wegovy averaged about 16.6% weight loss, while Foundayo’s highest approved dose averaged 11.1% over 72 weeks. Cross-trial comparisons are imperfect, but semaglutide currently has the edge on raw numbers.
- Dose steps: Foundayo has six tablet strengths; the Wegovy pill has its own escalation ladder.

Foundayo orforglipron weight loss results
In the 72-week trial the FDA relied on for approval (adults with obesity or overweight, without diabetes), average weight loss scaled with dose:
- 5.5 mg daily: 7.4% (about 16.8 lbs)
- 9 mg daily: 8.3% (about 18.7 lbs)
- 17.2 mg daily: 11.1% (about 25.2 lbs)
- Placebo: 2.1% (about 4.8 lbs)
Lilly’s Phase 3 ATTAIN-1 trial reported a 12.4% average at the top dose, along with improvements in waist size, triglycerides, non-HDL cholesterol, and blood pressure. Results also take time: participants lost roughly 5% of body weight by week 12, with the rest accumulating gradually over the following year. Foundayo dosing: the six-step schedule
Foundayo is taken once daily, swallowed whole, and increased gradually so your stomach can adjust. Each step lasts at least 30 days:
- 0.8 mg once daily (starting dose)
- 2.5 mg once daily
- 5.5 mg once daily
- 9 mg once daily (if needed, based on response and tolerability)
- 14.5 mg once daily (if needed)
- 17.2 mg once daily (maximum)

What if you miss a Foundayo dose?
Take it as soon as you remember, but never take two tablets in the same day. If you’ve missed seven or more days in a row, contact your healthcare provider before restarting, since you may need to step back down a dose. Foundayo side effects
The side-effect profile looks like the rest of the GLP-1 class, because the mechanism is the same. The most common are nausea, abdominal pain, heartburn, constipation, headache, gas, diarrhea, bloating, vomiting, tiredness, indigestion, belching, and hair loss. Most are mild to moderate, show up during dose increases, and ease as your body adapts. Foundayo carries the class-wide boxed warning about thyroid C-cell tumors seen in rodent studies, and shouldn’t be used by anyone with a personal or family history of medullary thyroid carcinoma or MEN 2. Rarer serious risks include pancreatitis, gallbladder problems, dehydration leading to kidney issues, and low blood sugar when combined with insulin or sulfonylureas. Two Foundayo-specific notes worth knowing:
- Birth control pills may not work as well. The label recommends a backup or non-oral method for 30 days after starting and for 30 days after each dose increase.
- Drug interactions matter more than with injectables. Strong CYP3A4 inhibitors cap the dose at 9 mg, strong CYP3A4 inducers should be avoided, and simvastatin is limited to 20 mg daily. Bring your full medication list to your prescriber.

How much does Foundayo cost?
Lilly sells Foundayo orforglipron through LillyDirect with self-pay pricing starting at $149 per month for the lowest dose and rising to roughly $349 per month at higher strengths. Eligible commercially insured patients may pay as little as $25 per month with a savings card. Since July 1, 2026, eligible Medicare Part D members can also access Foundayo for $50 per month through the Medicare GLP-1 Bridge program. Prices and programs change often, so verify before you fill. Is Foundayo orforglipron right for you?
Foundayo makes the most sense if you strongly prefer a pill over an injection, if the Wegovy pill’s fasting routine doesn’t fit your mornings, or if a daily habit is easier for you to keep than a weekly one. It may be a weaker choice if you want the largest possible weight loss (tirzepatide injections still lead), or if you take medications that interact with it.
